Primary Care
Those individuals on the HDHP, at appointment check-in please present your Bellin Health access card.
Physical Therapy & Urgent Care
At appointment check-in, please present your Bellin Health access card, a valid photo ID, and your insurance member card to the front desk. If a copay is required, it will be collected at that time. Failure to present your access card may result in your personal insurance being billed.

Fastlane Drive-Thru
Fastlane provides services for patients of all ages, offering a fast, convenient and climate-controlled drive-thru option for testing, vaccinations and other services.
Appointments are required and can be made by your provider, through your MyChart account or by calling the employer clinic hotline at (800) 528-7883.
FastCare
Cost: $0 copay due at time of appointment
Receive care for:
- Bladder infections (females ages 12 and older)
- Cold symptoms
- Earaches
- Immunizations
- Insect bites
- Screenings
- Sinus infections
- Sore throats
Patients must be 18 months or older. Additional age restrictions for select services.
Urgent Care
Cost: $75 copay due at time of appointment
Receive care for:
- Broken bones
- Colds
- Cuts requiring stitches
- Influenza
- Other ailments that are not life threatening
Excludes imaging.
