Community Health Implementation Plan (CHIP)
Our Community Health Implementation Plan (CHIP) is a comprehensive effort to create a healthier future for the people in our community. We work with local health departments and non-profit organizations to identify the community’s primary health issues and consider external factors affecting health.
We use proven prevention methods to improve population health and help people take charge of their well-being. We can identify community strengths, existing resources and potential gaps by developing action plans. With a solid commitment to our plan, we continue to make a meaningful impact.

Preventive Measures
Individuals who manage their health and prevent sickness can improve the community's overall wellness. In addition to addressing immediate health concerns, Bellin Health focuses on educating the community about the precautionary health measures they can incorporate into their lives to stay well. We prioritize early detection and disease awareness initiatives, such as educational cancer prevention events for the population we serve, to promote the importance of healthy lifestyle choices.
Cancer Prevention Events
Skin Cancer Awareness & Prevention - 2023
A nurse practitioner with Bellin Health's cancer program met with health students at West De Pere High School to discuss sun safety and skin cancer prevention. Prior to the event, sun safety and skin cancer quizzes were sent to the health teacher to assess student knowledge of the subject. Once the quizzes were returned, educational material was developed, along with a 'Jeopardy!' game to review the talking points.
Packers vs Cancer Presented by Bellin Health - 2023, 2022
This initiative was aimed at raising awareness about all cancers. Together, the Packers, Bellin Health and the Vince Lombardi Cancer Foundation collaborated in an effort to educate the public and Packers fans about the importance of early cancer screening and detection.
Beat the Pack - 2021
Partnering event with Bellin Employer Clinic for Fincanteri Marine Group. One-on-one smoking cessation coaching sessions with employees at three different sites for eight weeks. A total of 22 classes were held in the six-month period of January through June of 2023. There was a total of 184 participants; 12 employees confirmed quitting smoking by the end of their program, and 20 more employees significantly decreased their use by half a pack or more.

By working together, we can identify the community’s specific needs and develop targeted interventions, offering conscientious solutions.
One key focus of our plan is to address social determinants of health. We understand that education, personal funds, housing and access to healthy food can affect population health. With these factors in mind, we can create sustainable changes by identifying and addressing underlying social aspects as an organization.
