Virtual Nursing for Schools
We are committed to making your students’ health our top priority. Our virtual nursing services allow students to access a registered nurse via a TytoCare virtual unit or by telephone.
For Parents & Guardians
This program provides your child with quick, virtual access to a registered nurse at school, allowing them to stay in school while receiving the medical attention they need, leading to improved health and educational outcomes.
Your child does not need to be a patient to access these services.
Important Consent Forms
There are three forms that you need to complete in order for your child to be eligible for these services. If these forms are not on file and your child is feeling sick, they will not be able to access a virtual visit with a nurse.
- Registration agreement – this form allows the nurse to record and document visit details using an electronic medical record. Before signing this form, please review our patient rights and responsibilities and notice of privacy practices.
English registration agreement
Spanish registration agreement
Hmong registration agreement - Communication consent – this form allows the nurse to communicate with you and a school staff member. It is very important that a legal guardian’s name and phone number are listed, or we will not be able to provide services.
English communication consent
Spanish communication consent
Hmong communication consent - School release of health information (FERPA) form – this form is for the school, and it protects your child’s education records.
English release of health information
Spanish release of health information
NOTE: It is important that all fields on each form are filled out appropriately. If they are not, it could result in your child not having access to this program.
Once completed in totality, please submit your forms.
Answers to Your Questions
What is a virtual visit like?
When your child’s school staff determines that your child should be seen by a registered nurse, they will initiate contact through a secure TytoCare unit or via telephone call. One of our nurses will then answer the request and begin the visit. The school staff member will remain present and facilitate the visit with your child and the nurse.
Will my child be diagnosed and/or prescribed medication?
This service is intended for the registered nurse to assess your child’s symptoms. If the nurse determines that your child needs further medical attention or a prescription, the school staff member will let you know.
Is there a cost for my child to use this service?
No, there is no cost to you, and insurance will not be billed.
What happens with my child’s health information?
Your child’s privacy is always protected. The nurse will document the visit in our secure Epic software. This means that your child’s healthcare provider will be able to access the visit information at a later date should they need to.
