Our care team will be by your side throughout your surgical journey, offering support and guidance. Rest assured we are here to provide you with the highest level of care.
Bellin Health Surgery Prepare is a modern, patient-centric approach to coordinating care for surgery patients like you. Our goal is to establish an optimal plan for care before, during and after your surgery to ensure the success of your procedure. Our team collaborates to create a directive for a positive experience at every stage of your surgical process.
We have a surgical care team of anesthesiologists, surgeons, primary care physicians, nurses, pharmacists, case workers, social workers and physical therapists working together to identify any risks you might face and address them before your surgery. This includes strategies to mitigate problems like high blood pressure, heart disease, diabetes, smoking and other acute or chronic conditions.
That’s not the only care our team provides. We also offer access to transportation services and support systems to help during your recovery after discharge. Whatever you need, our team is ready to care for you every step of the way.
Meet Our Prepare Team
Your Prepare provider may be a physician, nurse practitioner or physician assistant. They’ll ensure you’re as medically safe as possible to have surgery. Your provider will also order any additional testing, refer you for specialty care and communicate their findings with your surgeon and primary care provider.
The nurse navigator is here to lead you through your operative experience. Think of them as your guide during the treatment process. You’ll meet your nurse navigator in the Prepare clinic.
If needed, a physical therapist will see you before surgery to teach you and your support person how to use devices to help you walk or manage stairs. They may also provide simple strength exercises for pre- and post-surgery healing.
We also have a case manager available to meet with you before surgery. They can help plan a safe discharge home, coordinate any community resources and services you might need, file paperwork like an Advance Directive or Power of Attorney and address any financial concerns.
