Blood Glucose Meters
Blood glucose monitors evaluate blood sugar levels throughout the day as advised by your healthcare professional. The gathered information can help adjust medication dosage, understand the impact of food and exercise on glucose levels, and pinpoint habits that may be leading to low or high blood sugar.
Devices for diabetes management are highly beneficial for improved self-care. Numerous devices can sync with your mobile phone. You can use the links below to check if your phone and its operating system are compatible with the mentioned devices. Additionally, these links allow you to share data with your healthcare team.
Glooko
Glooko helps you understand your diabetes information from all your devices: blood sugar monitor, CGM, pump and smart pen. You can locate all devices compatible with Glooko here.

Glooko Registration
- Visit the Glooko registration page and enter all required fields.
- In the Code field (labeled optional), enter the proconnect code: bellin > Click register. This allows for sharing with Bellin Health.

- Install the app > Launch the app > Sign in using your username and password (provided during account creation) > Synchronize your devices: blood glucose monitor, CGM, insulin pump, and/or smart pen.
- Begin monitoring your diabetes-related data such as: blood glucose levels, CGM, diet, physical activity, and medications.
Glooko Website
- Navigate to Glooko Uploader.
- Choose either Mac or PC based on your system.
- Once the installation file is downloaded, find and execute it.
- If you're on a PC: check your recent downloads folder. If you're on a Mac: check your downloads folder in the finder app.
- After finding the file, double-click it and follow the instructions displayed on the screen to install the software.
- A system restart will be necessary after the installation is complete.
If your devices do not connect via Bluetooth, you can transfer data from a blood glucose (BG) meter, insulin pump, or continuous glucose monitor (CGM) using Glooko's upload utilities. You will require the respective device maker's cable for this purpose.
You can contact the meter manufacturer (their phone number is found on the back of your meter) to request a data transmission cable.

Continuous Glucose Monitors (CGM)
Continuous Glucose Monitoring (CGM) provides real-time glucose readings that update every five minutes. Besides the glucose level, it also displays the speed and trend of change, as well as triggers warnings for low/high and increasing/decreasing rates. This information aids in making decisions about diet, physical activity, and medication. The data supplied by CGM enables you to actively control glucose levels instead of having to address low or high glucose levels as they arise.
The following are continuous glucose monitoring devices (CGM). These CGMs gather data non-stop through a sensor that is positioned on the abdomen or the back of the arm, based on the approved site location. This information is then sent to a mobile app or a separate reader/receiver.
Dexcom
Our medical center will send you a distinct sharing code via email to access your data, ensuring constant availability and streamlining your appointments. If you haven't received an invitation to share your data, you can request your diabetes educator or endocrinologist to provide you with a unique sharing code by sending them a MyBellinHealth message. You have the option to stop sharing with our clinic whenever you wish.
To upload, view, and store glucose data, as well as print or email reports from your home to our clinic, follow these steps:
Mobile App
Sign in to the CLARITY mobile app. Click on "share" and adhere to the provided instructions.

- Sign in to the CLARITY website.
- Follow the provided instructions.
Dexcom Receiver
- New CLARITY users need to register an account.
- Follow the guidelines to install the Uploader software.
- Connect your receiver to your computer for automatic upload.

Freestyle Libre
By inputting our practice ID (refer below) into the Libre software system, we can maintain uninterrupted access to your diabetes information, thereby streamlining your visits. You have the option to cease sharing with our clinic whenever you wish.

Insulin Pumps & Smart Pen
Conventional insulin pumps administer insulin at predetermined rates set by the prescribing healthcare professional. The user declares their pre-meal glucose levels and the amount of carbohydrates they plan to eat, and the pump then instructs the user on the amount of insulin to administer or "bolus." The user then chooses to deliver the bolus, at which point they receive an insulin dose to cover the meal and any necessary correction for the pre-meal glucose level.
Insulin pumps are made up of a reservoir that is filled with fast-acting insulin, linked to an infusion set that attaches to the body, and an internal catheter that infuses insulin into the body. The latest developments in insulin pump therapy include the Artificial Pancreas and Hybrid Closed Loop Models. These pumps contain an algorithm that automatically adjusts the basal rate delivery every five minutes, based on sensor glucose levels and active insulin previously delivered as bolus insulin.
Tubeless Insulin Pumps
A tubeless insulin pump is available for purchase, which attaches to the body via an inbuilt reservoir loaded with insulin. It is water-resistant and can be used for up to three days, offering a continuous insulin supply for 72 hours. The pump is paired with a personal diabetes manager that resembles a smartphone, used for programming the pump.
Medtronic CareLink

Tandem t:connect
Mobile App
- To download the mobile app, go to Google Play or the App Store and select t:connect mobile.
- To activate, select mobile link, unlock the pump, then go to Options > Device Settings > Bluetooth Settings.
- Toggle the switch to enable the Mobile Connection and activate Bluetooth. If Bluetooth Settings is not available, visit the Customer Portals to update your pump to the latest software (a prescription and training might be necessary).
- Log in to the mobile app using your t:connect web app credentials (or create a username and password in the t:connect mobile app). For best results, upload your pump data to the t:connect web app via a USB cable before starting the next pairing process (USB instructions provided below).
- To connect your Insulin pump to the mobile app, follow the instructions to pair your t:slim X2 insulin pump with your smartphone. Note that only one device can be linked with the t:connect mobile app at a time.

USB Connection
- Configure and upload data from your Tandem pump via the supplied USB connection.
- Send a MyBellinHealth message to your healthcare provider to request a Tandem t:connect invite, if you're not already sharing data with them.

Omnipod/Glooko
Configure and transfer data from your Omnipod pump, to Glooko software via the supplied USB link to your computer.
- Activate the Glooko ProConnect Portal on your smartphone.
- Launch the Glooko mobile app. Navigate to Settings > Account > Add ProConnect code: bellin. Afterwards, inform your provider or diabetes educator that you have transferred data to Glooko.
If you're using the original Omnipod (as opposed to the Omnipod Dash), please reach out to the technical support number below and choose option 2 to request a complimentary cable for data upload.
