The goal of our lifestyle medicine care teams is to use personalized, evidence-based support to be your expert partner in creating and sustaining long-lasting nutrition, sleep, physical fitness, stress management, social and substance use habits that are the foundation for improving your overall health, happiness and quality of life. Lifestyle medicine is a medical approach that focuses on preventing, treating and even reversing chronic conditions by addressing the root causes, your daily habits.
The Six Pillars of Lifestyle Medicine
We believe there are six key pillars that, when addressed and improved through building positive habits and routines, aid in achieving an overall healthy lifestyle. Should you have a chronic condition that requires an additional level of specialized care, we will work with those care teams to ensure you are receiving the care and treatment you need.
Lifestyle Medicine & Primary Care
We understand that navigating your care journey can be overwhelming, and that is why we are committed to being there for you through it all. We recognize that our lifestyle medicine program may seem similar to the preventative care you receive from your primary care provider. However, there are different reasons each specialty is essential to improving and maintaining your health and why they complement each other.
Our lifestyle medicine providers and care teams provide high-touch, continuous communication with you to improve your everyday habits and behaviors related to our six pillars in collaboration with your primary care provider and their focus on preventing and treating health conditions.
Your journey with our lifestyle medicine team begins with a free intake session that includes a brief introductory video and questionnaire that helps us understand your health history and goals.
Then, you will meet one-on-one with one of our nurses who will take the time to learn about the outcomes you are looking for and recommend how to start achieving them.
After your time with one of our nurses, your personalized visits with our care team will begin. This will start with a series of appointments with a lifestyle medicine provider, a registered dietitian and a health coach. Each visit is dedicated to various pillars of lifestyle medicine and helping you achieve comprehensive wellness. During these visits, you will establish your care with our team, and we will continue to work closely with you, designing and adapting your care plan that provides the resources you need based on your goals and the right level of support, accountability and guidance.
Your lifestyle medicine care team will be made up of a group of collaborative, expert providers who focus on each pillar that makes up your overall health and well-being, creating long-lasting, well-rounded habits and behaviors.
Registered Dietitians
Your registered dietitian is specifically dedicated to your nutrition and how it impacts your overall health, including:
- Assistance building balanced meals that fit your daily life
- Ongoing, frequent support, including help adjusting as needed
- Personalized nutrition guidance that is tailored to your health conditions and goals
- Practical strategies for eating at restaurants
Health Coaches
Your health coach focuses on helping you develop long-lasting, real-life habits through your day-to-day behaviors through:
- Building encouragement and motivation
- Helping to create sustainable routines
- Identifying obstacles
- Providing tools to manage challenges you are experiencing
Lifestyle Medicine Providers
Your lifestyle medicine provider is responsible for overseeing your total health journey, including:
- Addressing the root causes of your existing medical conditions
- Collaborating to develop a personalized, sustainable care plan
- Delivering high-touch care, including more frequent and longer visits
- Managing your medication(s)
