Remote Patient Monitoring
Remote Patient Monitoring engages you in your own care by giving you hospital-grade tools to learn condition-specific education, monitor vital signs and symptoms and connect with your care team. Our team focuses on empowering you to live an independent, healthy life.
How does it work?
Patients enrolled in this program are given a daily care plan to follow using a tablet, scale and vital sign equipment provided by our team. Every day, patients will answer health-related questions and record their vital signs, including blood pressure, temperature, weight, heart rate, blood oxygen level and more depending on their individual care plan.
A registered nurse reviews your information daily, identifies any changes in your vital signs or condition and collaborates with you and your provider to make any necessary updates to your medication(s) or care plan to ensure you get the care you need, when you need it.

Our Infusion Services team of licensed pharmacists and infusion nurses work with you and your care team to coordinate intravenous (IV) referrals to ensure a smooth transition home, determine appropriate home infusion devices and make progress toward your healthcare goals.
