- Another convenient payment option for anyone to use is Quick Pay. Please utilize this service if your statement is from Bellin Health Home Care Equipment.
- For billing questions, help understanding your statement or to make a credit card payment by phone, contact our Business Office at (920) 445-7210. If you would like an itemized copy of your statement, we will gladly provide it upon request.
Price Transparency
Wisconsin Act 146 (2009) tells healthcare providers in our state how to give you price and quality information. You have the right to request information regarding the median billed charges and publicly available quality of care metrics.

We highly recommend you utilize the estimate tool found at MyBellinHealth for more accurate information about what your insurance will pay and your out-of-pocket costs. It is important for you to know that these charge documents do not contain any information about your out-of-pocket cost for your treatment. The posted files reflect the overall contractual discount we provide to the various insurers. The rate listed is based on our accounting system and not specific to the code listed.
Insurance Coordination
All patients must present their current insurance card(s) and will be asked to assign insurance benefits to Bellin Health. Your insurance may require you to get prior approval and to complete necessary claim forms before receiving services.
Your insurance contract is between you and the insurance company. As a courtesy to you, we will be happy to prepare and submit a claim to your insurance carrier. You are responsible for all charges not covered by your insurance plan. You must pay the total amount if your insurance company doesn't pay your claim within a reasonable time.
Your out-of-pocket costs, including copays or deductible amounts, will vary depending on the health insurance policy you selected and how it covers the treatment you seek. Please be aware that the estimate of out-of-pocket costs is only as accurate as the information your health insurance company makes available to us.
Accidents and Injuries
As a patient, you are responsible for medical expenses incurred due to a motor vehicle accident or liability. However, as a courtesy to our patients, the hospital will bill your health insurance, government programs and liability insurance.
Credits and Refunds
The National Health Insurance Industry determines how insurance companies divide the payment on your account. We will check insurance overpayments with the insurance companies and refund them by following state and federal rules. To inquire about a credit on your account, please get in touch with our billing department.

Payment Arrangements
Our financial advisors will help you find the best payment plan for charges not covered by insurance. Information is available in our Financial Assistance Policy.
You can pay with cash or a with credit card at the Patient Registration Department. You may make a payment before or after your hospital visit. Accepted credit cards include Visa, MasterCard and Discover.
If you cannot pay within 30 days, please call (920) 445-7210 to approve and schedule payment arrangements for up to 24 months. Financial assistance is available to all patients with financial needs.
Worker's Compensation
