The Black Tie & Blue Jean Extravaganza is an annual, one-of-a-kind fundraising event that takes place each fall and is located in the Lambeau Field Atrium where guests are invited to a wonderful evening that benefits a different Bellin Health program or service each year.

Presenting Sponsor
As the presenting sponsor, you will receive media promotion, recognition at the event, recognition in the invitation, a corporate table with signage and 10 reserved seats at the event.
Cost: $10,000
Diamond Sponsor
As a diamond sponsor, you will receive media promotion, recognition at the event, recognition in the invitation, a corporate table with signage and eight reserved seats at the event.
Cost: $7,500
Gold Sponsor
As a gold sponsor, you will receive recognition at the event, recognition in the invitation, a corporate table with signage and eight reserved seats at the event.
Cost: $5,000
Table Sponsor
As a table sponsor, you will receive a corporate table with signage and eight reserved seats at the event.
Cost: $2,000
Individual Ticket
An individual ticket gets you one seat at the event.
Cost: $150
