Advance Care Planning is a process that helps you reflect on your healthcare wishes, select a trusted person to make choices for you if needed and make a written plan for the future.
What is an Advance Directive?
An Advance Directive is a legal document where you identify your healthcare wishes, request any special ceremonies or traditions and name your power of attorney to make decisions for you in the event you cannot make them for yourself.
We recommend everyone over the age of 18 has an Advance Directive. Family members do not have automatic legal authority to make healthcare decisions on your behalf if you are not able to.

Access Advance Care Planning Tools on MyBellinHealth
After logging in to your MyBellinHealth account, you can select "Advance Care Planning" from the Health Menu to check your Advance Directive documents on file and upload any missing documents.
After uploading a new file, a Bellin Health team member will review it and send you a message through MyBellinHealth once it has been accepted or if more information is needed.
