Sports Injury Assessment
To schedule a video visit that can be done from the comfort of your own home, please select a Location, then "Video Visit Orthopedic Injury Assessment" Visit Type.
To schedule a visit to come into our facility, please select a Location and then any of the other appointment types under Visit Type to be seen in person by our LAT providers.
To schedule your appointment online:

- Select the visit type
- Select Submit
